(a) (1) A licensee who fails or refuses to comply with a request for the dental records of a patient, that is accompanied by that patient's written authorization for release of record to the board, within 15 days of receiving the request and authorization, shall pay to the board a civil penalty of two hundred fifty dollars ($250) per day for each day that the documents have not been produced after the 15th day, up to a maximum of five thousand dollars ($5,000) unless the licensee is unable to provide the documents within this time period for good cause.

Best dentist I've ever had! Dr. Kolchansky is hands down the best in Simi. Don't take your chances going anywhere else. Dmitri's (office manager) professionalism is remarkable. He thoroughly explains insurance info and coverage. He is prompt, efficient, and very knowledgeable. Overall, an outstanding group of people work there and you will not be disappointed.

Great service and always explain everything the dentist does. Excellent office staff and great offices and great parking. Always on time and great work. Always briefs you on how much your payment will be after insurance. Set a plan on what needs to be done on your teeth. Always friendly! Great service.

After having a dentist for over 20 years, it's hard to find one that you can see yourself going back to because you are so comfortable with the old routine. After a few struggles with other offices, I finally found Dr. K, him and his entire staff are amazingly attentive and friendly. He is very throughouh with you, tells you exactly what you need in a realistic time frame. I've never been more comfortable with an office! Thank you so much Dr. K and team! I can't wait to start bringing the whole family!
