Meet Our Doctor

Dr. Cook was raised on a small farm in Brazil, Indiana. He received his Bachelor of Science with high distinction from Indiana State University. He then attended Indiana University School of Dentistry, where he graduated with high honors. Dr. Cook completed a two-year residency at James Witcomb Riley Hospital for Children, where he received a certificate in pediatric dentistry and a Master of Science in Dentistry. During his second year of the program, Dr. Cook had the honor of being Chief Resident of the pediatric dental program.

We are committed to building relationships that offer personalized care and oral health education. We strive to provide quality dentistry that families can be confident in and trust. Dr. Cook reserves some Thursday mornings and some Fridays to treat patients in the hospital in Evansville, Indiana. The office will be closed on weekends and major holidays.
Financial
Report
Unless otherwise previously arranged, payment is expected as services are performed. When treatment is required, you will be given an estimate and will be asked to make definite financial arrangements with a member of our business office staff. This will lessen confusion and provide better understanding.
